Age UK - Whitstable - Free 10 Hour Home from Hospital Scheme

Age UK Whitstable's Home from Hospital scheme is designed to provide support, both practical and emotional, for older people who have been discharged from hospital.

The scheme is open to anyone over the age of sixty who is a resident in the Whitstable CT5 postal area.

The Home from Hospital scheme aims to aid the transition from a stay in hospital back into everyday life.
