Hedera House Supported Living

Hedera House is a supported living service for adults with learning disabilities, specialising in dual diagnosis. Emphasis is placed on supporting individuals to develop their social skill and on empowering them to take control over their own lives. This is achieved through tailored support plans, which are developed by the individual with support from people such as their key worker, advocate, local specialist teams and/or care manager.

People are supported to live as independently as possible, taking part in social and leisure activities that enrich their lives and build on independence.

Notes

Each Tenant has access to 24hr staffing support.

Emphasis is placed on supporting individuals to develop their social skill and on empowering them to take control over their own lives. This is achieved through individuals support Plans, which are developed by the individual service users with support from people such as their key worker, advocate, local specialist teams and/or care manager. These plans are regularly reviewed and amended, as the individual’s needs dictate. Formal reviews are held within a multidisciplinary context six weeks after admission and on a three to six monthly basis thereafter. All decisions and plans are recorded in consultation with those present at the reviews and are implemented accordingly.

Individuals have the opportunity to state who will be involved in process of individuals Planning and their Key Worker has specific responsibility for ensuring that agreed programmes are implemented effectively. For those individuals with mental health problems the individuals support plan will be reviewed in line with the care.
