Acting as a guide through the health and social care system, Social Prescribers connect people with appropriate community resources and support services, with the aim of enabling people to increase their independence and ability to self-manage, thereby reducing demand on health services. People being discharged from or who have recently left hospital will receive 12 weeks of support to meet identified and additional needs.

Social prescribers offer impartial information, advice and guidance through home visits, telephone or by email. Taking a personalised approach, the Social Prescriber works with the individual and family to develop a holistic support plan, which is then worked through in partnership to enable the person to achieve their desired outcomes, goals and aspirations. To do this, the Social Prescriber will connect people with organisations who can provide ongoing help.
Housing, financial support, form filling, home safety (including aids and adaptations/ assistive technology), befriending, social activities, transport, domestic support, respite and signposting to domiciliary care.
