Hook Lane Supported Living

Hook Lane is a supported living service for adults with with learning disabilities and autism spectrum conditions. Hook Lane is able to support individuals from the age of 17 in transition from residential college, foster care or the family home.


Support is structured to ensure that the individual has the opportunity to access a full range of activities within the community including employment opportunities and access to local colleges where possible in order to increase independence and make the transition as smooth as possible.

Notes

We Offer:

  • 24-hours a day by shared support; including night support
  • 1:1 support for specific identified assessed needs which vary from person to person
  • Full inclusive needs support planning, care needs and risk assessments by a well trained staff team, experienced with working with adults with Learning Disabilities and Mental Health needs.
  • Our aim is to decrease 1:1 support hours as skills are further developed and greater independence gained where possible
  • Support is structured to ensure that the individual has the opportunity to access a full range of activities within the community including employment opportunities and access to local colleges where possible.
  • An energetic and enthusiastic, trained staff team.
  • Supporting individuals in accessing appointments with all health care professionals.
  • Outreach support for those able to move on providing continuity of support.
